Canadian customers, to receive your gift card:

  1. Click on the image of the claim form below to open and print the form.
  2. Complete the required information on the form.
  3. Enclose a copy of your receipt dated between April 1, 2012 and
    June 30, 2012 with the purchase price circled. (hand written or altered receipts will not be accepted).
  4. Enclose the original UPC (barcode) from the package.
  5. Send to:
